Jennifer M. Demma, MSN, APRN-CNM, FACNM

Jennifer Demma

Personal Profile: Jen has been a midwife since 2000 and has worked in a variety of hospital and community-based practices in the Twin Cities. She practices from a foundation rooted in the philosophy of midwifery, equity, and justice.  She is committed to bodily autonomy, body liberation, cultural humility, and the ongoing journey of self-awareness.  Jen is passionate about reproductive and sexual health and providing person-centered, holistic, compassionate, trauma-informed, non-shaming, and affirming health care.  She also has a strong interest in education and has been a faculty member at Metropolitan State University, MN State Mankato, and Georgetown University.  When not working, Jen is usually cuddling her cats and watching TV or movies and on the best of days enjoys walking, connecting with friends, escapes to Lake Pepin, anything related to creative expression, and the quest for delicious food.

Personal Profile: Liesl first started at Family Tree Clinic as a volunteer, then as a registered nurse for over five years, and is thrilled to continue to be at Family Tree in the role of family nurse practitioner. Liesl believes everyone has the right to quality healthcare and that to be patient-centered means to understand systems of oppression and intersectionality and partner with patients to make healthcare accessible and individualized. She is committed to providing trauma-informed, compassionate, empowering care. When not working, Liesl enjoys hiking, knitting, going to concerts, playing soccer, and spontaneous trips to the North Shore.

 

Kadie Fredell, APRN-FNP

Kadie Fredell

Personal Profile: Kadie has been a registered nurse since 2009 and a nurse practitioner since 2017.  She believes quality healthcare is a human right and is committed to caring for underserved populations.  She loves getting to know her patients and sharing her knowledge of how our bodies work so that they can make informed decisions that are best for them.  When not working, Kadie enjoys gardening, cooking and seeing live music.  She also loves anything that gets her outdoors, including camping, canoeing, and hiking with her Australian Shepard.

Kelsey Leonardsmith, MD

Personal Profile: Dr. Kelsey first became excited about working with trans and gender-expansive youth as a medical student in Boston. She has worked at many community health centers in St. Paul and is passionate about caring for folks who are not always served well by our healthcare system. Some of her areas of interest are in caring for gender-expansive youth, getting nerdy about hormones, options for non-binary transition, pregnancy care, and sexual health. She loves being outside in all weather, believes in cultural humility, speaks Spanish, and is passionate about using her tools as a physician to work toward a more just world.

Personal Profile: Jenny started at Family Tree Clinic as a health educator while a student at Hamline University and eventually returned as a provider at the clinic in 2014. Jenny is deeply committed to providing high-quality, compassionate, patient-centered care, and is excited to be part of the dedicated, committed team at Family Tree. When not working, Jenny enjoys spending time with her family and friends exploring the outdoors.

Lexy Millett, MSN, CNM

Lexy Millett

Credentialed specialty: Certified Nurse Midwife 
Personal Profile: Lexy is a Certified Nurse Midwife who joined the medical field because she enjoys building long-term relationships with people and partnering with folks to support their health goals. She has worked in both large hospitals and small birth centers, but the community clinic setting is her passion because she believes that every single person deserves high-quality health care. She frames her practice through the lenses of reproductive justice and the dismantling of racism in healthcare, intentionally working to partner with folks who have been ignored by the traditional healthcare system. Outside of work, Lexy enjoys spending time with her partner and kiddo, canoeing, hiking, and eating sweet treats.

Phyllis Wiener, APRN-WHNP

Credentialed Speciality: Women’s Health Nurse Practitioner

Education:BA from University of MN education and urban studies; RN from Minneapolis Community College, NP certification Planned Parenthood, RNC, NP

Personal Profile:Phyllis Wiener (she/her) has been a nurse practitioner for 30 years. Phyllis started her career at Cedar Riverside People’s Center, and spent 20 years working at Planned Parenthood as both a healthcare provider and educator in the nurse practitioner certification program. In 2012 she joined the team at Family Tree Clinic. At Family Tree Phyllis provides reproductive and gender-affirming care to adolescent and adult patients. As a provider, she partners with patients to focus on their goals and priorities through deep listening, engaged participation and supporting individuals to reach their goals. She is committed to working with patients to decrease barriers, and improve their lives and healing through affirming health care practices. Phyllis lives in Minneapolis with her partner Shay, has two adult children and a 2-year-old puppy she loves to take for walks around the lakes.

Nora Powers, PA-C

Personal Profile:Nora (they/them, she/her) first started with Family Tree as a volunteer and was excited to return as a provider. They are a native of the Twin Cities and have worked in several community clinics in the area.  Nora strives to offer healthcare that helps patients live their best, most fulfilled lives. They are deeply committed to bodily autonomy, reproductive access, and understanding gender outside of the cishetnormative binary. Outside of work, they love meeting other people’s pets and cooking delicious food from their unruly garden.
