Maria Kaefer, MD

Personal profile: Dr. Kaefer graduated from Southern Illinois University School of Medicine and completed her residency training at the Lawrence Family Medicine Residency in Lawrence, Massachusetts in 2002. She subsequently worked for four years at the University of Minnesota and at a variety of community clinics in the Twin Cities and Oakland, CA.Her special interests include holistic medicine and empowering patients to take an active part in their own health and healing. She is fluent in Spanish and committed to cross cultural care, care of the underserved, and the fight for universal health care coverage.
Rachael Berube, NP

Personal Profile: Rachael has been a registered nurse since 2011 and a nurse practitioner since 2013. She loves partnering with her patients to help them achieve their health goals, and is especially passionate about providing care to women and queer people. When she is not at work, Rachael is usually spending time with her with her baby daughter, Iris. She also enjoys cooking, reading books, drinking coffee and being with friends.
Jennifer M. Demma, MSN, APRN-CNM

Personal Profile: Jennifer has been a midwife since 2000 and has worked in a variety of hospital and community-based practices in the Twin Cities. She is committed to partnering with clients to provide individualized, holistic, compassionate health care and is thrilled to be part of the amazing team at Family Tree. Jennifer has a strong interest in education and has been a faculty member at Metropolitan State University, MN State Mankato, and Georgetown University. When not working, Jennifer enjoys walking around the lakes with friends and anything related to the local arts scene.
Lauren Radziejewski, MSN, APRN-CNP
Credentialed specialty: Adult Nurse Practitioner

Personal Profile: Lauren is an Adult Nurse Practitioner and the newest medical provider to join the staff of Family Tree. Originally from New York, she comes to us after practicing at Mayo Clinic in Rochester where she was an Instructor of Medicine, was appointed to the Division of Internal Medicine and maintained a special clinical practice within the Department of Endocrinology, co-founding Mayo’s Transgender and Intersex Specialty Care Clinic. Throughout this experience and during her many educational speaking engagements, Lauren has continued efforts toward improving transgender healthcare and advocacy.
Lauren holds a Bachelor of Arts degree from the University of Richmond in Studio Art. After college, she worked in the arts for several years before returning to school to earn her Bachelor of Science degree in Nursing from Virginia Commonwealth University. She then went on to receive the Master of Science in Nursing from VCU and is currently completing her Doctorate in Nursing Practice from the University of Minnesota. When not caring for patients Lauren is an avid craftsperson, artist and world traveler. She is fluent in English and Spanish and has enough French to order a baguette or find the nearest Metro stop.
Liesl Wolf, BSN, PHN, RN

Education: Saint Catherine University, Saint Paul, MN: BS in Nursing, BA in Sociology
Personal Profile: A former Family Tree Clinic volunteer, Liesl is excited to be serving as Nurse Manager. Liesl is passionate about providing patient-centered care that is inclusive, accessible, and empowering. When not working, Liesl likes to go to concerts, play soccer, hang out with her two sisters, travel near and far, and explore the outdoors.
